OOSH service provides support to younger people with disabilities to participate in meaningful activities with their peers.

Aims of Lifeskills Plus Inc. Out of School Hours Service

To enable young persons with disabilities to :

  • facilitate interaction with a wide range of people
  • undertake similar recreational/sporting/social activities in which their peers participate
  • allow access to the local community and to facilitate the integration with disabilities into their community.


Service

  • Referrals can be accepted from schools, carers , health/welfare agencies and people in the community.
  • A meeting with the coordinator for an assessment is arranged – this is to have the service guidelines explained, to ascertain if the person is eligible for the service
  • If eligible and there is a vacancy within the service a person will be offered assistance to the end of the current financial year (at this stage the needs of each person receiving a service and those on the waiting list are reviewed and those with the highest needs will receive a service for the next financial year),
  • An ISP (individual service Plan) will be developed with the person with a disability, their family/carer/advocate.
  • An ISP defines individual goals of person with disability within OOSH guidelines.


Eligibility

To be eligible for Out of School Hours support a person must:

  • Reside in the Mid-Western Regional LGA
  • Have a physical, intellect8ual, sensory disability or a combination of any of these.
  • The person must be permanent, or likely to be permanent, and has resulted in a substanhtially7 reduced capacit7 for learning, communication or mobility;
  • Be enrolled at school


Priority

Priority is given to those people eligible for service who:

  • Are 12 years if age and over
  • Have the highest support needs
  • People who have no or limited access to other services for reasons of social isolation, geographic location and lack of peer support networks.
