The Transition to Work is a two year program which aims to support and improve employment outcomes for school leavers who can transition to work within
1-2 years. The service assists school leavers with a
disability to develop skills that will help move them into employment, vocational education and training or higher education
Aims of Transition to Work Service
are to ensure that clients of the service are
supported :
- In making the transition from school to adult life.
- By having pre vocational skills and work readiness promoted to them
- By providing opportunities to participate in a variety of work placement settings.
- By being given opportunities to learn about OHS Rights and Responsibilities of
Employees and Employers
- By being given opportunities to develop greater independence and decision making skills
- With assistance with personal care and physical mobility where required.
Service
Before commencing with service a Transition Plan is developed between client, school, family and Lifeskills Plus staff.
Independent Service Plan (ISP) is put together within 3 months with client, family ,
advocate etc and documents personal goals.
Activities are based on an assessment of persons prevocational and work readiness skills but may include the following:
Independent living skills 9 literacy, numeracy, computer skills); prevocational training (work ethics, interview skills, protective behaviours) and work experience placements.
Eligibility
To be eligible for the Transition to Work
Service a person must be either:
- Currently receiving individualised Transitition to Work funding from DADHC
- A school leaver with moderate to high support needs who is 18 years of age before 31 December or has completed year 12
- Has an intellectual, physical or sensory disability who is eligible for a service under the NSW Disability Services Act (1993) and is assessed as eligible by DADHC
- Is a resident of NSW or is approved to transfer from Community Participation Program and is
- NOT undertaking paid employment (for more than 15 hours per week), full time further education or higher education.
- Early or late entry is decided on a case by case basis only in exceptional circumstances by the Regional Director.
Service Area
The program will support eligible people with disabilities in the Mid-West Regional Council Local Government Area.
Transport
Transport may be provided during the day to assist a person to access the daily program of activities, but it is not provided for commuting purposes at the beginning or end of a day other than in limited circumstances.
